Adoption matters are Special Proceedings filed before the Clerk of Superior Court. Adoption petition forms are available from the Special Proceedings Division. The Department of Social Services has compiled assorted checklists to help you ensure all necessary forms have been completed for an adoption. Adoptions can be extremely complex and all demand high attention to detail, and it is recommended that you seek competent legal counsel to assist you.
